Continuing Care Plans for Ongoing Support
At Gage and Associates, we provide individualized Continuing Care Plans for clients who may benefit from ongoing support after treatment. These plans are developed prior to discharge, ensuring a smooth transition and setting you up for continued success in recovery.
Each Continuing Care Plan is tailored to your unique needs and may include:
- Suggested support group participation
- Referrals to community resources
- Additional recovery-focused activities
Continuing Care Based on Risk Classification
Minimal and Moderate Risk:
Continuing care is
not typically required.
Significant Risk:
Continuing care
may or may not be recommended, depending on your case. However, if you plan to attend a
formal or informal hearing with the Illinois Secretary of State, you will likely need to provide:
- A copy of your Continuing Care Plan
- A Continuing Care Status Report
- Proof of completion (if applicable)
High Risk:
A
formal Continuing Care Plan is required before discharge. For Secretary of State hearings, you must demonstrate:
- Substantial behavioral progress
- A minimum of 12 months of sobriety
- An established and active support system
